SKILLED NURSING SERVICES (SUPPORT)
Our skilled nurses provide specialized medical assessments, treatments, and therapies designed to improve patient outcomes while maintaining their safety at all times. Our nurse practitioners have advanced knowledge in areas like end-of-life care; wound care; pain management; IV therapy; respiratory therapy; infections control; nutrition management; therapeutic exercise programs etc., depending upon the needs of our clients’ diagnosis or condition. Skilled nursing visits are provided for up to 7 days per week if needed
RESIDENT CARE (HOME AID)
At 7th Grace Oak Homecare, we understand how important it is for seniors and people with disabilities to stay in their own homes and receive the necessary care they need. Our home health aides are able to come into your home and provide a variety of services, such as companionship, light housekeeping, meal preparation, medication reminders, and shopping assistance. Our aides are screened for experience, knowledge, and compassion. We ensure that each of our aides is dedicated to providing quality care that meets each client’s individual needs.
NURSING CARE (OUR CARERS)
7th Grace Oak Homecare offers top-quality nursing care by employing licensed professional nurses (LPNs). Their role includes but is not limited to observing & assessing patient condition regularly, monitoring vital signs, administering prescribed medications, and teaching self-care techniques. Our LPNs have vast experience dealing with diverse cases, from normal pregnancies to complicated illnesses requiring 24-hour service. Whether it’s short-term coverage or long-term commitment, our licensed professional nurses are always available round the clock to provide the highest quality medical attention.

At 7th Grace Oak Homecare, our highly skilled team provides hourly visits for medical assistance, personal grooming, assistance with meals, or just companionship. For those who require more frequent support, we have specially trained teams that can provide daily visits as well as live-in care services.
Bathing & Grooming Assistance
Our caregivers understand the importance of hygiene and will help with bathing, dressing, grooming, and oral hygiene tasks such as brushing teeth and shaving. We also help with make-up application when necessary or desired. Personal hygiene is vital for physical health as well as self-esteem! Our team is always available to answer any questions you may have about the process.
Medication Reminders
We understand the importance of taking medications correctly at the right time every day – it’s crucial for maintaining good health. Our team will help ensure that medications are taken on time by providing reminder services as needed throughout the day.
Meal Preparation
At 7th Grace Oak Homecare, we understand how crucial proper nutrition is to overall health and development - particularly for seniors! We can provide meal planning advice and prepare meals per dietary restrictions or preferences specified by our clients or their families. We also assist with grocery shopping if desired.
Companionship & Safety Supervision

We also provide safety supervision when needed – whether it be during regular activities like walking outdoors or going out for errands – we’re here to provide peace of mind.
Light Housekeeping & Laundry
For those unable to take on household chores themselves due to age-related physical limitations, our compassionate staff can help manage light housekeeping tasks such as Vacuuming/sweeping/mopping floors, dusting furniture, washing dishes/laundry, changing linens, etc., always with dignity and respect.
Morning & Bedtime Assistance
For those needing extra help getting up in the morning or settling into their bedtime routine, our experienced staff can assist by providing an extra set of hands when required – helping them dress appropriately / use bathroom facilities safely / brush their teeth/ wash their face /take medications, etc. Just let us know what you need, and we’ll do our best to accommodate your wishes.
Pick Up Prescriptions
At 7th Grace Oak Homecare, we understand how difficult it can be at times getting out of the house, so we’re happy to offer prescription pick-up service when requested – hassle-free! Just let us know where you want us to pick up prescriptions from (pharmacy/doctor)and when you need them picked up.
Registered Nurse Assistance & Plan Of Care Assessments
For those requiring additional medical attention beyond basic home care needs, our Registered Nurse (RN) can create individualized plans based on each client’s unique situation – after thorough assessments have been made in order to meet specific needs. The goal is always to improve quality of life while remaining independent at home.
Physical Therapy
Our physical therapy service is focused on improving mobility, reducing pain and restoring physical function. Our trained physical therapists work with patients to develop personalized treatment plans that include exercises and other therapies to improve strength, balance, coordination, and range of motion.
Occupational Therapy
Designed to help patients regain the ability to perform daily activities such as dressing, cooking, and personal hygiene, after an injury or illness. Our occupational therapists develop treatment plans to improve motor skills, cognitive function, and sensory integration, to enable the patient to perform these activities with greater ease.
Speech Therapy

24 Hour Care
In cases where round-the-clock care is required due to severe medical conditions, we offer 24-hour live-in care services whereby professional caregivers stay overnight at residences providing continuity of care whenever needed. This allows people suffering from degenerative diseases like Alzheimer’s to receive regular monitoring.
